RESIDENTIAL SCHOOL SURVIVOR REGISTRATION FORMThe Truth & Reconciliation Commission is asking all former residential school students to register by completing this form.

All registered former students will be given special identifcation and will be provided one (1) meal for each of the following days: June 16, 17, and 18th.

*There is NO REGISTRATION FEE to attend the National Event. We are inviting and encouraging you to register! Participants must cover their own expenses for travel and accommodations.*

The Truth and Reconciliation Commission will have statement gatherers present to record the statements of those who wish to to share their Residential School experiences with the TRC.

STATEMENT GATHERING AT THE NATIONAL EVENTThis process is completely voluntary and you are free to tell the TRC statement gatherers about any aspect before, during or afer Residential School that you wish. You are welcome to bring someone to the recording with you.

You may also wish to access the services of a Health Support Worker. Health support workers provide emotional support, cultural support and professional counseling. If you wish to speak to a Health Support worker prior to the National Event please call the Health Canada Manitoba Region phone line at 1-866-818-3505. This number will connect you with a health support worker in your local area (make sure this includes NW Ontario). These workers will be in attendance at the National Event and will be available to help throughout the entire event.

We also encourage you to bring any photographs, documents or other materials that you wish to share with the TRC. The TRC will scan or copy your documents and return them to you.

The Winnipeg National Event is an outdoor event at The Forks and Parks Canada so please dress appropriately (i.e. wear proper footwear, use sunscreen , bring a hat and be prepared for all kinds of weather) Participating in an outdoor event can present dangers and please be mindful of the outdoor space and any potential risk .
You agree to voluntarily assume all risks of bodily injury.

You also assume all risks for loss or damage to your personal possessions.

You release The Forks and Parks Canada, its employees, the Truth and Reconciliation Commission and its Commissioners, employees and contractors from any claims and liability whatsoever in connection with any injury or personal harm you may sufer, as well as in respect of any loss or damage to your personal possessions you may incur, in preparation for, during, or afer the Event.

You also indemnify The Forks and Parks Canada, its employees, the TRC and its Commissioners, employees and contractors from all losses, damages and claims made by a third party which arises in whole or in part from any your acts or omissions while in preparation for, during, or after the Event.
